If you are currently registered with another Licensed Producer and would like to order our products, you can transfer your existing Medical Document in two simple steps:
Send an email to your current licensed producer and CC: firstname.lastname@example.org. Let them know that you would like to request a transfer of your Medical Document to Opticann. They will need to fax your prescription directly to our secured fax line at: 437-826-9005. We will help follow up on your request.
As soon as our Patient Care Team has received and processed your documents, you will receive a new client verification email from us indicating you can begin placing orders. In the meantime, please ensure to register as a new patient with us to begin the process.
