Opticann by Heritage has partnered with ReLeave Therapeutics to offer our patients a quick and easy way to connect with a Healthcare Practitioner to see if medical cannabis is right for you.
ReLeave’s team of general practitioners, pain specialists and psychiatrists are compassionate, and knowledgeable experts, with one goal in mind: to help people. Their nurse practitioners share this goal and are there to help support you through the intake process and follow-up process, as well as provide cannabis education.
How to Get Your Medical Document
What is a medical document? A medical document is a form (similar to a prescription) that is completed by a physician or nurse practitioner which authorizes a patient’s use of medical cannabis in Canada. If you do not have a healthcare practitioner, we have made the process easy to quickly obtain your medical document online through our trusted partner clinic.
Follow these 3 easy steps to get started.
There is no fee and no doctor referral is required. Telehealth services are fully covered by your provincial health plan. Have your provincial Health Card ready for the intake form.
Talk to a Physician
A licensed physician from Releave will call you to discuss if medical cannabis is right for you, and help answer any questions on formats and dosing. They will submit your completed medical document.
What to Expect from Your Consultation
A telemedicine consultation typtically takes approximately 15 minutes. This can be done from the comfort of your own home or anywhere you can find a quiet, private space.
The physician will determine your eligibility then fax in your completed medical document to Heritage Patient Care team.
Once we have received both your registration and your signed medical document, you will receive your unique patient ID number (Client ID) via email from Heritage. You can now log into the Heritage Client Portal and begin ordering Opticann medical cannabis products.
