Here are the steps to take for your patients to successfully register and order.
Patient Registration
The patient must submit their registration application using our online registration form or downloadable form to create an account.
All of the patient’s personal information (full name, date of birth, residential address, etc.) on the registration form MUST match with the Medical Document.
Healthcare Practitioner Prescribes
If the patient is eligible for medical cannabis after consultation, complete the Medical Document, outlining the prescription details.
Physician or licensed healthcare practitioner must send the original medical document to Opticann Patient Care via secure fax: 437-826-9005.
Confirmation
Opticann by Heritage verifies both patient documents. Our Patient Care team will process the patient documents as quickly as possible. We will notify the patient via email and/or phone call notifying them their account is now active.
This usually occurs within the same day or next business day.
Patient Orders
Your patient can now order products from our Patient Portal by signing into their account with their Client ID sent via email. Or if they prefer, they may also order products over the phone by calling Patient Care at 1-855-437-2266.
Opticann’s full CB4 product line is available to purchase online from the Heritage Patient Portal.
Orders are shipped via Canada Post with 1-2 day delivery nationwide.
